SmartCase: Interactive Clinical Case Reports

In the era of guideline-driven medicine, we can increasingly appreciate the importance of case reports as an educational tool. Case reports showcase how experienced clinicians navigate complex scenarios, apply clinical judgment, and adapt general guidelines to individual patient needs. They may describe unusual etiologies, challenging differential diagnoses, rare disorders, novel therapeutic approaches or novel adverse reactions to care.
It's my firm belief that clinical case reports should be a pleasure to read and learn from for all clinicians.
Just as NEJM already successfully uses case reports as a springboard for CME activities, I believe we can enhance the learning utility of all case reports. The advent of large language models has made it increasingly feasible and cost-effective to enhance case reports for clinician education.
For instance, through a multi-agent AI framework, we can extract key clinical pearls and learning objectives from each case report. From these, we can generate questions that promote active recall, widely recognized as one of the most effective learning strategies. Advanced retrieval-augmented generation (RAG) methods enable us to identify related research articles and, conversely, to accurately assess the novelty of submitted manuscripts.
